The Colville Tribal Diabetes Program’s mission is to provide the highest quality, culturally competent health education services to each community in a collaborative manner which promotes a safe and healthy environment. Our program focuses on providing preventative education tools for people through advocacy of healthy lifestyles that honor mind, body and spirit.

This form will be completed by Diabetes Prevention staff in coordination with medical personnel to enroll you in services.

Download and open the form in Adobe Acrobat. On the side menu, choose Fill & Sign. Complete these forms and save to your computer, then email as an attachment to staff OR print, fill in by hand and then send completed form via fax to 509.634.2819.

Services

For any services you will need current diabetes prescription, be an enrolled member of a federally recognized tribe, and be eligible for Indian Health Services.

You can receive these services 1 time per year.
